Supporting Documents Required:

1. Father's valid Lebanese Passport with a recent Family Civil Registery Record (Ikhraj Kayd)
2. Mother's valid Lebanese Passport and Birth Certificate if she is not Lebanese
3. Original child's birth certificate + the two copies.
4. Lebanese marriage certificate for parents
5. Self-addressed, stamped envelope
6. To add child's name to one of his/her parent's passport requires a notarized letter of authorization from parents and a fee of $10.00 money order, per name, and 2 passport size pictures (Both notarized)
