Marriage
- Fill out a complete Marriage Application
- Original Marriage Certificate & Marriage License plus two copies
- Husband/Spouse Lebanese ID Card, A new Family Extract of Registration اخراج قيد عائلي حديث a recent Individual Civil Registry Record (Ikhraj Kayd) plus first three pages of his passport.
- Spouse’s Birth Certificate ( If she is not Lebanese) plus a copy of her passport.
- Copy of the Green Card or US Passport.
- Self Stamped return address envelope.

Divorce
- Fill out a complete Divorce Application
- Original Divorce documents( Final Decree) + Notarized copy translated into Arabic language, & Legalized by the office of the Great seal Secretary of State.
- Lebanese Marriage License وثيقة الزواج اللبنانية الصادرة عن المديرية العامة للاحوال الشخصية
- Husband/Spouse Lebanese ID Card, a recent Individual Civil Registry Record (Ikhraj Kayd), A new Family Extract of Registration اخراج قيد عائلي حديث plus a copy of the passport.
- Spouse’s Birth Certificate, plus one notarized copy ( If she is not Lebanese).
- A Proxy (Special Power of Attorney) is required to execute the Divorce in Lebanon.
- Fees: 30$ Money Order payable to the Consulate General of Lebanon in Detroit.
- Self Stamped return address envelope.

- Lebanese Marriage Certificate plus one copy.
- To add child’s name to one of his/her parent’s Lebanese passport, a notarized letter of authorization from parents, 2 passport size pictures (notarized) are required and $15.00 money order payable to the Consulate General of Lebanon in Detroit.
- Self Stamped returned address envelope.

- Fill out a complete Citizen Application
- 2 copies of a A new Family Extract of Registration اخراج قيد عائلي حديث
- Notarized statement from the wife requesting the Lebanese citizenship. Spousal approval should be included in the same statement
- 2 copies of the wife's Birth Certificate
- ِAn Affidavit from The Religious Authority (Priest, Pastor, Or Imam) stating that the Couple lived & still living together.
- 2 copies of the wife's passport showing the change in her last name
- 2 copies of the Lebanese passport of the husband
- 2 copies of the Marriage Certificate
- $20.00 money order payable to the Consulate of Lebanon
- Self-addressed stamped envelope
