Passport (navy Blue colour)
Fill
in the passport
application form completely, in English and in Arabic
Enclose the
following:
- Your passport and copies of
the first 6 pages
- Three colored photos
- A current Individual
Extract of Origin issued after 1993 and one copy
- A copy of the driver's
license/ or Green Card
- Fees:
- ** Fee: $200.00 for 5
years (thumb print on white card) paid by money orders only
- ** Fee: $40.00 for 1 year
(thumb print on pink card) paid by money orders only
- A self-addressed stamped
envelope for the return mail. For faster and secure return of your
documents, it is recommended to use Certified Mail, Express Mail or
other express courier like FedEx, UPS, DHL...
- Your right thumb
fingerprint on the White or Pink card provided by your Embassy or
Consulate within your jurisdiction. This card must be submitted with
your application. Place your right thumb fingerprint on the appropriate
box in front of the Notary Public. The Notary Public should notarize and
indicate on the back of the card or on a separate sheet of paper that
the fingerprint is the right thumb of the bearer.

Adding
child/children name(s) on passport
- The father of the child
should be Lebanese
- The child should be registered
with the Consulate
- Notarized Birth Certificate
of the child
- A copy of your driver's
license
- Notarized letter from the
parents authorizing the Consulate to add their dependent to one of their
Lebanese passports
- One 2x2 photo
- A money order (no cash or
check accepted) in the amount of $15.00 per child payable to the
Consulate of Lebanon
- Certified Self-addressed,
stamped envelope

send.

Renewal
of Palestinian Travel Document
- For holders of travel
documents issued after January 1, 1993, the applicants should:
- Fill in passport
application form
- Two 2x2 photos (notarized
from a notary public)
- UNRWA card (Bitakat Iashi)
plus Tathkarat al-Hawiya
- Copy of the Applicant's
Green Card
- Copy of the Applicant's
Drivers License
- A money order (no cash or
check accepted) payable to the Consulate of Lebanon: $200.00 for five years
- Certified Self-address,
stamped envelope for return
N.B.
"White card form" should be submitted with the application. Please
contact the Embassy or the Consulate within your jurisdiction and ask for the
"White Card Form"
This card should have the print of your right thumb. Put your right
thumbprint on the appropriate box in front of a notary public. The Notary
public should notarize and indicate on a separate sheet of paper that the
fingerprint is the right thumbprint of the bearer.
- For holders of travel
documents issued before January
1, 1993, the applicant should:

- Send copies of: the travel
document, UNRWA card and the Palestinian ID
- Granting the renewal is,
exclusively, based on the approval of the Lebanese Security General
- Two 2x2 photos (notarized
from a notary public)
- Copy of the Applicant's
Green Card
- Copy of the Applicant's
Drivers License
- Certified
Self-addressed, stamped envelope

Please allow a period of two to three months for the reply
